Everyone providing instruction for the Sparkman High School Band must fill out the appropriate paperwork. Below is a list of documents that must be submited 30 days prior to any instruction.
- I9 – ONLY PAGE 7 IS NEEDED. Requires a photo copy of your drivers liscense and social security card.
- Contract for Services – Contracts are usually sent to you by the director via mail or email.
Please use one of the following three methods to return these documents to me.
- Print and fill out documents. Rescan them and email to David Raney or Jack Albert.
- Print and fill out documents. Mail them to:

I would prefer method #1 for a much quicker processing. If you have any trouble with any of these methods please feel free to contact me. When you recieve the contract through email or mail please inform the director which method you will be using to return the documents.
